How do I refer a child?
Make-A-Wish accepts referrals for potential wish children from parents or guardians, medical professionals – or even the children themselves.

What are our criteria?
A child needs to be a UK resident, aged between 3 – 17 years of age, fighting a life-threatening condition and should not have received a wish from another wishgranting organisation. We can accept a child if we receive the referral on their 3rd birthday or before the day of their 18th birthday.
Please remember to have the Medical Release form signed – this will allow us to write to the child's Consultant as soon as possible, in order to establish medical eligibility. When you have signed it, please put it in an envelope and return it to:
Make-A-Wish Foundation UK,
329-331 London Road,
Camberley,
Surrey

What happens next?
After confirming the child has a life-threatening condition, we will send a short form to the family asking for further information about the wish child. Once we have this form back we will arrange for our specially-trained volunteers to visit the wish child to find out what their wish is.
